| Services to Elderly and Disabled People Aims To enable elderly and disabled people to achieve and maintain an independent lifestyle of their own choice, with any necessary support and assistance from community, professional and voluntary services. This should be achieved by helping people to live with dignity in their own homes or where this is not desired or possible, in locally based acceptable alternatives. Service Objectives To individually assess and plan for meeting the needs of people who may need services. To individually assess and plan for meeting the needs of carers for people who use our services. To work closely with partner organisations, service users and carers in planning for local services. To develop and promote a range and choice of quality services through the process of contracting, quality control and monitoring. To provide information about the range of services and their access. To ensure that within the available financial resources, services are provided cost effectively and are provided to those in greatest need. |
